The information provided should contain as much background and detail as possible but please do not exceed three (3) pages typed, excluding this form and the letter of endorsement, with single spacing within paragraphs, double spacing between paragraphs, and bold-face headings for various criteria.

II. SELECTION CRITERIA
Induction into the SD Cooperative Hall of Fame is the highest honor that the cooperative community bestows on those men and women whose endeavors in the cause of the cooperative form of enterprises have been GENUINELY HEROIC. The SD Cooperative Hall of Fame provides recognition for a person contributing in especially significant ways to the enhancement of the cooperative idea, its broader acceptance or to the substantial advancement of cooperative enterprise in any of its various forms.
In making its decision, the Selection Committee considers the following criteria within the environment in which the nominee lived and worked. In order to help the Committee make its selection, please provide detailed information on the major contributions and activities of the nominee relating to these criteria as well as the individualís commitment and contributions to his/her community. Application should not exceed three (3) typed pages using single spacing within paragraphs, double spacing between paragraphs, and bold-faced headings for various criteria.
1. Demonstrated statesmanship, innovation, personal commitment, leadership and vision beyond the professional competence and requirements of the position for which he/she was employed to serve within the cooperative sector. If nominee did not hold office in the cooperative movement, please indicate how he/she demonstrated the same qualities to advance cooperatives. (25 points)
2. Achieved identifiable and lasting changes to improve and promote cooperatives. Inspired and
persuaded others to act to advance the cooperative system. (15 points)
3. Commitment to serve the cooperative community through years of service, membership and offices held. (15 points)
4. Individual commitment and contributions to his/her community. (15 points)
Points are awarded for each of the categories listed above, placing emphasis on cooperative activities.

V. DEADLINE FOR NOMINATIONS
Nomination and letters of endorsement must be received at the office of the SD Cooperative Hall of Fame Committee no later than June 16, 2017. Nominations and letters received after the deadline will not be considered this year but will be considered the following year.

VI. SELECTION POOL
Nominations are held in the selection pool for two consecutive years
Inductees selected for inclusion in the SD Cooperative Hall of Fame will be recognized at the 2017 Co-op
Month Banquet to be held September 13, 2017, at the Ramkota in Watertown, SD.
